"CareBridge team on AMD Developer Hackathon: ACT III Hackathon"
Team Idea
CareBridge AI is an AI-powered wellbeing companion that supports people between human care encounters. It provides lightweight check-ins, identifies meaningful changes in wellbeing over time, helps users prepare for upcoming appointments, and creates structured summaries for care professionals. When uncertainty or concern is detected, it knows when to stop and guide the user toward human support. For the AMD Hackathon, we are building the core reasoning layer on AMD infrastructure, using an open model for conversation understanding, longitudinal change detection, uncertainty assessment and pre-visit summarisation. Our goal is not to replace healthcare professionals, but to make the time between human encounters safer, more useful and better connected.
