NOAI+: AI reads confidential files, names hidden

Confidential files read by AI, patient files first, then contracts. NOAI+ hides names, IDs, phones, addresses, birth dates and listed medical terms before text leaves, puts them back in the answer and signs a receipt. Runs on Gemma 4 on AMD GPUs.
