"ClearDischarge team on AMD AI League Hackathon"
Team Idea
ClearDischarge turns confusing hospital discharge papers into instructions patients can actually follow. A patient or nurse uploads the discharge note, and an open AI model running on AMD GPUs (AMD Developer Cloud) rewrites it at a 6th-grade reading level in Arabic or English. It builds a daily medication schedule, highlights “go to the ER if…” warning signs, and checks understanding with three teach-back questions. Anything unclear or missing from the original note is flagged for a nurse to review. It never adds new medical advice; it only explains what the doctor already wrote.
