Medication non-adherence affects an estimated 50% of patients with chronic conditions worldwide, leading to avoidable hospitalizations, disease progression, and preventable deaths. Existing solutions rely on paper prescriptions, generic phone alarms, or memory alone — none of which provide structured tracking, feedback loops, or visibility for healthcare providers. Adhera is a web-based medication adherence platform designed to close this gap for patients managing chronic conditions and the providers who care for them. Patients register their medications with flexible schedules (daily, specific weekdays, alternate days, or as-needed) and receive automated reminders via email and browser push notifications at each dose slot — Morning, Afternoon, Evening, or Night. With a single tap, they mark a dose as Taken, Missed, or Snoozed. Every interaction is logged in an append-only, tamper-proof history. Patients can also submit side-effect reports with severity grading from 1 (Mild) to 4 (Emergency). A Severity 4 report immediately triggers an alert to the assigned provider and emergency contacts within 60 seconds. Healthcare providers get a real-time dashboard showing all assigned patients, their overall adherence rates, and critical alerts for anyone falling below 70% weekly adherence. They can drill down into per-patient medication history, review feedback records, and export adherence reports as PDF or CSV. Administrators manage account creation, verify healthcare provider registrations, handle patient-to-provider assignments, and maintain a full audit log retained for five years. What makes Adhera unique is its combination of patient simplicity (one-tap responses, zero app install required) with clinical-grade data integrity (append-only records, role-based access, Row Level Security at the database layer) and real-time emergency escalation. It is built with FastAPI, Supabase (PostgreSQL 15+), React 18, and deployed on Vercel — fully open source and production-live.
