AfterWard - Checked by the ward, clear at home
Leaving hospital should be the beginning of recovery, not the start of confusion. Today a discharge ends with a printed letter: the ward hopes it is complete, the patient hopes to understand it, and every doubt turns into a phone call, a wasted trip or even in instructions not properly followed. AfterWard accompanies the whole discharge, on both sides. For the ward, it starts before the handover. AfterWard reviews the bundle the hospital is about to deliver and points out what a reader will miss: a sheet that is mentioned but not attached, a dose or a date that is missing, two instructions that contradict each other, a sentence that only a doctor would understand. The professional corrects, completes or explains in a few clicks, keeps full control of every decision, and shares a bundle that is ready for real life. For the patient and the family, it continues at home. The documents arrive already organised, with each original sentence explained in everyday words, faithful to the numbers, dates and conditions the doctor wrote. A relative can help with their own access. Whatever is still unclear becomes one organic and persistent chat with the ward, instead of scattered calls and messages. It closes the loop: the ward sees the requests in one place, answers directly or with a draft to review, and each answer stays linked to the question it resolves. The patient can ask for a clarification, send a reminder, and declare the doubt resolved. Nothing is lost, nothing is left hanging. Patients whose hospital does not use AfterWard yet can start from their own documents and still get the same clarity. We believe a discharge is not finished when the letter is printed. It is finished when it gets home complete and is understood at the kitchen table, the morning after. What happens after the ward is decided by clinicians. AfterWard makes sure it comes true.
